Nomination FormNominations for an award should normally be made within 2 months of the incident. The main exception to this is cases involving legal proceedings, where we appreciate a delay is unavoidable. If the rescue took place in Merseyside, Lancashire, Cheshire or the Port of Liverpool, you should contact the Liverpool Shipwreck and Humane Society. We've drawn up a separate form for each main type of rescue. Please select the one that applies to your nomination. Fill it in and print it out. Then send it to us at the address on the left. If you are from the police, fire, ambulance service or HM Coastguard, please send the completed nomination form to your Awards Department at HQ. They will then send it on to us.
